FireMed Memberships
Membership agreement
To
enroll or renew your membership online, please read and accept or decline
this membership agreement at the bottom of this page. For more information,
call the Air Life membership office at
1-800-353-0497.
- Household membership includes all persons who are
permanent residents of the same single-family dwelling/noncommercial
residence living together as part of a family unit, but not to include
roomers or boarders. Membership benefits are also extended to include
household members living in substitute care (e.g. nursing homes) within
your city and district ambulance service areas. The first person listed
on the application form is designated as the “Primary Member.” Anyone
who joins a household after the membership goes into effect can be included
under the membership 72 hours from the date the “Primary Member” notifies
Membership Services of the addition. Only those persons who meet the
membership eligibility requirements and are listed in the membership
record at the time services are rendered are eligible for benefits.
- FireMed and/or Air Life membership is secondary to
insurance carriers. We will accept payment from insurance carriers as
payment in full.
- I transfer directly to the FireMed
agency and/or Air Life my rights
to ground and/or air medical insurance payments due me. Such payments
shall not exceed the regular charges of FireMed and/or Air Life’s regular
charges.
- Ground memberships are honored by FireMed Membership
Programs of Oregon. Ground membership covers ground ambulance charges only.
- Air Life memberships are honored by the Association
of Air Medical Membership Partners (AAMMPs) reciprocal transport group.
Air Life membership covers air ambulance charges only.
- New member benefits take effect 72 hours after receipt
of completed enrollment form and payment.
- Both ground and air emergent medical transports
are based on medical need, not membership status, and transport
patients to the closest medically appropriate facility, as requested
by physician. Nonemergency transports are not covered by this agreement.
- Availability of service cannot always be guaranteed due to weather conditions, commitment to another transport, or mechanical out-of-service.
- No refunds will be issued on membership purchases.
